KEEP YOUR UPDATED VIAL OF LIFE FORM IN YOUR REFRIGERATOR
Have your medications changed? Had another surgery? Change in health insurance or primary care physician?
If so, stop into the Senior Center and pick up a new form. We can help you fill it out if needed. Don’t have a Vial Of Life? We can provide one for you, too. Stop in, we’ll be happy to help! And also remember that we have a card for your vial notifying the emergency personnel that you have a pet being left home alone, should you need it.
The vial, paperwork and instructions are available, free of charge, at the Senior Center. Be sure to keep the information on your form current.
